CALMS
Memory loss is an enormous strain on everyone in a family, including those suffering from a form of Dementia. Dementia refers to a gradual deterioration of mental functioning, such as concentration, memory, and judgment, which affects a person’s ability to perform normal daily activities.
Dementia occurs primarily in people who are over the age of 65, or in those with an injury or disease that affects brain function. While dementia is most commonly seen in the elderly, it is not a normal consequence of the aging process.
The Accessible Home Health Care CALMS program was created to provide:
* Certification of Caregiver staff as appropriate and qualified to provide care for patients….
- Verification documents to determine the proficiency levels of AHHC Caregivers
- In-service materials made available to Caregivers by owners from other sources
- Guidelines for caregiving
- Appropriate activities for caregivers to perform individualized to each patients needs.
* All CALMS patient information will be carefully documented on the:
CALMS Care Plan and weekly activity log
-Administer a Reminder Program promting patients to:
Take medication at the prescribed times.
Practice good hygiene
Toilet prior to incontinence
Establish a schedule for eating meals
Remind of family relationships & events
Prevent wandering and calm anxeity
